The state requires the filing of Form W-2 if there are any wages paid to employees, even if there is no state tax withholding during the year.
Indiana State ID Format:

10 digits space followed by 3 digits. (Eg: 9999999999 999)

10 digits employer TID followed by 3 digit Employer TID Location number(Eg: 9999999999-001)
Filing Criteria and Requirements
Criteria 1: If there is any Indiana state tax withheld

Filing Method: Direct State Filing

You must file a W-2 directly with the state.

Additional Form Required: Form WH-3.
