The following forms are used for authorization purposes

• Form 8453-EMP: If you’re filing an original or amended employment tax return through an ISP and/or transmitter and you’re not using an ERO with an e-filed return, you must file Form 8453-EMP. 
• Form 8879-EMP: If you’re an ERO, e-filing tax returns, you can either use Form 8453-EMP or Form 8879-EMP to obtain authorization to file an employment tax return.
