Form 1099-R is an IRS tax form used by Financial institutions that have made a distribution of $10 or more from retirement plans, annuities, profit-sharing plans, individual retirement arrangements (IRAs), and other similar types of accounts, and benefits during the tax year. The "R" in the form number stands for "retirement."

Payments of reportable death benefits made by employers must also be included on Form 1099-R, as well as any reportable disability payments.
