If the field "Name of covered individual(s)" has a value, and the corresponding check box "Months of Coverage" is not checked for months ranging from January to December, then the corresponding "Covered for all 12 months" must be checked.
What does it mean:
As long as you have entered "Names of covered individuals" in part IV of Form 1095B, you must at least check the "Covered for all 12 months" checkbox or the corresponding "Months of Coverage" checkbox.
