The IRS Form 941 has been updated multiple times over the past year due to COVID-19; it has been updated once again for the second quarter of 2021.

There has only been one worksheet for calculating the refundable and non-refundable portions of paid leave credits and the employee retention credits until this quarter. 

For the second quarter of 2021, the following Form 941 worksheets should be used:

Worksheet 1 - This worksheet is used to calculate the refundable and non-refundable portions paid sick and family leave wages. 

Worksheet 2 - This worksheet is used to calculate the refundable and non-refundable portions of Employee Retention Credit for the wages paid after March 31, 2021, and before July 1,

Worksheet 3 - This worksheet is used to calculate the credits on sick and family leave wages paid for the leave taken after March 31, 2021, and before October 1, 2021 

Worksheet 5 - This worksheet is used to calculate the COBRA Premium Assistance Credit.
