What is Form 15397?

Form 15397, also known as Application for Extension of Time to Furnish Recipient Statements, is used to request a one-time extension of up to 30 days to furnish copies of information returns to your recipients.

When can you request Form 15397?

  • Form 15397 can be used to request an extension of up to 30 extra days for filing recipient statements, such as Forms 1099 and W-2, when you need more time to complete the filing.
     
  • You must file Form 15397 by the due date of the recipient statements, but not before January 1st. Your request must be received no later than the date the statements are due to the recipients. If approved, you will generally receive a 30-day extension to furnish the statements.
