Follow these instructions to complete the form step by step:
1. Specify Submission Method
Choose how you will submit your annual reconciliation and Form W-2s:
- Electronic Submission: Use Louisiana’s LaWage application for electronic filing.
- Physical Media Submission: Submit your reconciliation and W-2s via CD or DVD using the specified record layout.
2. Complete Employer Information
Enter the following employer details on Form L-3:
- Business Name and Employer Identification Number (EIN)
- Contact Information, including address and phone number
3. Reconcile Payroll and Withheld Taxes
Fill out the following sections to reconcile the Louisiana income tax withheld during the year:
- Line 1: Enter your total gross payroll for the calendar year.
- Line 2: Report the total amount of Louisiana income tax you previously remitted during the year. Ensure this matches the amounts reported on Form L-1 filings.
- Line 3: Provide the total number of employees and the number of W-2s submitted with this form.
- Line 4: Enter the total Louisiana income tax withheld, as calculated from each W-2 form.
Note: If the totals on Lines 2 and 4 do not match, you must amend the corresponding Form L-1 filings to address discrepancies and remit any additional taxes due or request a refund for overpaid taxes.
