What is Form NC-4?

Form NC-4 Employee’s Withholding Allowance Certificate is a North Carolina state form used by the employer to withhold the correct amount of state income tax from the employee's pay.  If the employee does not submit Form NC-4 to your employer, your employer must withhold the filing status as “Single” with no allowances.
