Which employer should file Worksheet 5?

As an employer, if you've provided COBRA continuation coverage to eligible employees, you can claim the COBRA premium assistance credit using Form 941, Worksheet 5. The refundable portion of the COBRA premium assistance credit will be shown on Line 13f of Form 941.
