What information needs to be provided in the BOI report?

Businesses should provide the following information in their BOI report:

  • Type of report - Businesses should specify the type of report, whether it's an
    • Initial report
    • Corrected report
    • Updated report or
    • Newly exempt entity report.
  • Reporting Company information
    • Name
    • DBA or Trade name
    • EIN
    • Address
  • Company applicant information
    • Name
    • Date of Birth
    • Address (If the applicant is responsible for filing the documents to register the reporting company with the secretary of state or a similar office should provide the complete business street address otherwise provide the residential address)
    • Unique identifying number
    • Acceptable document images such as state driver’s license or passport. 
  • Beneficial owner information
    • Name
    • Date of Birth
    • Residential address
    • Unique identifying number
    • Acceptable document images such as state driver’s license or passport

Note: If the company applicant or beneficial owner has obtained the FinCEN ID, they can provide it in the BOI report instead of providing all the information.
