The State of New Jersey mandates the employers and insurers who provide Minimum Essential Coverage (MEC) to NJ residents to file Form 1094-B/C and 1095-B/C with the state and distribute the form copy to the corresponding recipient.

Due Date
- The deadline to file 1095 forms to the New Jersey Division of Taxation is March 31.

Mailing Address
The State of New Jersey does not accept the paper filing of ACA 1095 forms. Therefore, employers and health coverage providers are required to e-file ACA forms with the State of New Jersey.
