[Error Code: 1095C-050-02] How do I resolve it?

Rejection message:
If Form 1095C 'BirthDt' in 'CoveredIndividual' has a value and no corresponding checkboxes in group 'CoveredIndividualMonthly' ('January' through 'December') are checked, then corresponding checkbox 'CoveredIndividualAnnual' must be checked.
What does it mean?
If you are a self-insured employer who checked the covered individual’s checkbox and entered at least one 'SSN' or ‘Birth Date’ against each covered individual’s name. You must then check the “Covered all 12 months” box against the individual’s if covered for at least one day in every month of the year or the corresponding months for which these individuals were covered. You must not leave them blank.
