What is Form 1095-B?

Form 1095-B, Health Coverage, is used by insurance companies and other coverage providers to report health coverage information to the IRS and taxpayers. It applies to individuals who enrolled in and received minimum essential health coverage outside of the marketplace under the Affordable Care Act (ACA).

The form includes details such as the months during which individuals (and their dependents) were covered, the effective coverage dates, and the coverage provider.
