Does an employee have to complete all of the steps in Form W-4?

No. The form has a total of 5 steps, out of which the employee must complete Steps 1 & Step 5.  In Step 1, the employee has to enter personal information like name and filing status, and Step 5, where the employee should sign the form. 

Steps 2 - 4 are optional and need to be filled only if they are applicable, but doing so will make the employee's withholding more accurate matching the liability.
